To expedite the processing of your application, please include the month & year you began employment/terminated employment and an accurate phone number (including area codes) of all former employers.
You will need to fill out your employment for the past 3 years (If a company you have worked for in the past has gone out of business, we will need a copy of your tax statements or a copy of your check stub to prove employment with them.)
You will also need to submit a photocopy of the following, fax to 952-890-8192, before you will be approved for orientation:
- Current Commercial Driver's License
- Long Form Physical
- Medical Card
- Social Security Card
